Notes:

1. Resident will pay electrical utility direct to Duke Energy Progress with monthly allowance provided by PHA.
2. Revision necessary to comply with PIH Notice 2014-12.
3. Increase in monthly rental amount shall be reduced to comply with 35% maximum annual increase.
